What is Xifaxan?

Xifaxan is a prescription medication that is classified as a rifamycin antibacterial.

Uses

What is Xifaxan used for?

Xifaxan is used for the treatment of traveler’s diarrhea (TD) caused by noninvasive strains of Escherichia coli in patients aged 12 and up. Rifaximin cannot be used to treat patients with traveler’s diarrhea who have a fever and/or blood in stool or are presumed to be infected with a different pathogen than Escherichia coli. Rifaximin can also be used to reduce the risk of recurrence of overt hepatic encephalopathy (HE) in patients aged 18 years and older. Xifaxan is also used for the treatment of irritable bowel syndrome with diarrhea (IBS-D) in patients aged 18 years and older.

How does it work?

Xifaxan binds to bacterial DNA-dependent RNA polymerase and blocks a step of transcription which inhibits bacterial protein synthesis and thus inhibits bacterial growth.

How do I take Xifaxan?

Xifaxan can be taken with or without food. Take this medication as prescribed, taking more of this medication is not advised.

Xifaxan is available by brand-only as 200 mg and 550 mg tablets.

What is the recommended dosage?

Xifaxan is typically taken three times daily for the treatment of traveler’s diarrhea or IBS-D and typically twice daily for hepatic encephalopathy but take as directed by your doctor. The recommended dosage for TD is 200 mg three times daily, 550 mg three times daily for IBS-D, and 550 mg twice daily for HE. Duration of therapy varies among the indications.

What is the maximum dosage?

The maximum recommended dosage is 1,650 mg/day.

What if I miss a dose?

Take the missed dose as soon as you remember unless it is close to the time you are due for your next dose. If that is the case, just wait until it is time for your next dose. Do not double your dose to compensate for your missed dose.

Warnings & Precautions

What should you know before you take Xifaxan?

Xifaxan has not been shown to be effective in treating traveler’s diarrhea due to pathogens other than Escherichia coli. If diarrhea worsens or does not improve in two days, discontinue and alert your physician.

There is a risk of acquiring Xifaxan-induced Clostridium difficile diarrhea which can be observed if diarrhea does not improve or improves and then there is an onset of diarrhea upon completion of therapy.

Use with caution in patients with severe hepatic impairment.

Use caution when using concomitantly with p-glycoprotein inhibitors.

What are the side effects of Xifaxan?

Side effects may differ between the different indications for Xifaxan use.

Common side effects for the treatment of TD include (≥2%):

  • Headache

Common side effects for the treatment of HE include (≥10%):

  • Swelling of lower extremities and/or abdomen
  • Nausea, dizziness, fatigue

Common side effects for the treatment of IBS-D include (≥2%):

  • Increase in alanine aminotransferase (ALT)
  • Nausea

Interactions

Does this medication interact with other drugs, foods, or alcohol?

  • Xifaxan can induce CYP3A4 but at the current recommended dosing regimen, this interaction is not expected to have a significant clinical impact unless the patient has reduced liver function.
  • An increase in systemic exposure of Xifaxan is observed when given with p-glycoprotein inhibitors.
  • Changes in INR have been observed when taking Rifaximin concomitantly with warfarin and more frequent monitoring of INR levels may be required.

How should this medication be stored?

Keep Xifaxan in the original container and store it in a dry place at room temperature 15-30°C (59-86°F).
