What is Combivent Respimat?

Combivent Respimat is a prescription medication that is classified as a combination of two medications, ipratropium (anticholinergic agent) and albuterol (beta2-agonist).

Uses

What is Combivent Respimat used for? 

It is used for the treatment of chronic obstructive pulmonary disease (COPD) in those who are currently taking a regular bronchodilator and continue to have bronchospasms.

How does it work?

It reduces bronchospasm through both anticholinergic and sympathomimetic mechanisms; the combination of medications work synergistically to have a greater bronchodilator effect than either medication on its own.

How to administer Combivent Respimat?

  • To prepare for use for the first time, press the safety catch while pulling off the clear base (do this with the orange cap closed). Do not touch the piercing element inside the bottom of the clear base.
  • Write the discard date down on the label. Discard the medication 3 months from the date the cartridge is inserted into the inhaler. 
  • Remove the Combivent Respimat cartridge from the box and push the narrow end into the inhaler. About ⅛ of an inch will stick out/remain visible if the cartridge is inserted correctly. Do not remove the cartridge once it has been inserted. 
  • You will need to prime (first time use only) your inhaler so that it will deliver 120 doses. 
    • Hold your inhaler upright with the orange cap closed. 
    • Turn the clear base towards the direction of the arrows until you hear a click. This will be about half a turn.
    • Flip the orange cap so that it snaps open fully. 
    • Point your inhaler toward the ground and away from your face. Predose the dose release button then close the orange cap. Do this until the spray is visible. This means your inhaler is now ready for use.
  • Daily dosing:
    • Hold your inhaler upright with the orange cap closed.
    • Turn the clear based towards the direction of the arrows until you hear a click. This will be about half a turn.
    • Open the orange cap until it snaps fully open. 
    • Breathe out fully and slowly, then close your lips around the end of the mouthpiece without covering the air vents
    • Point your Combivent Respimat inhaler towards the back of your throat.
    • While taking a deep, slow breath through your mouth, press down on the dose release button and continue to breathe in slowly as long as you comfortably can. 
    • Hold your breath for ~10 seconds or as long as you comfortably can. Close the orange cap until it is time to use your inhaler again. 

If it has been over 3 days since you have used your inhaler, spray 1 puff towards the ground to prepare your inhaler for use. 

If it has been more than 21 days since you have used your inhaler, spray puffs until a spray is visible.

What dosages are available?

Combivent Respimat is available as an oral inhalation (spray) in ipratropium bromide  20 mcg/albuterol 100 mcg per actuation. 

Generic ipratropium bromide and albuterol for nebulization is available as 0.5 mg and 2.5 mg per 3 mL.

What is the recommended dosage?

Recommended dosing is 1 inhalation four times daily.

What is the maximum dosage?

Max dosing is 6 inhalations in 24 hours.

What if I miss a dose?

Take the medication as soon as possible if you have missed a dose. If it is almost time for your next dose, skip the missed dose and return back to your regular schedule. Never double the doses.

Warnings & Precautions

What should you know before you use Combivent Respimat?

A paradoxical bronchospasm and potentially be life-threatening can occur with use of an inhaled bronchodilating agent.

Combivent Respimat can cause dizziness and blurred vision. Caution with performing tasks requiring mental alertness. 

Caution in patients with sensitivity to sympathomimetic amines. 

Do not exceed the recommended dose or frequency or use with other medications containing a LABA due to serious adverse effects and potential fatality.

Hypersensitivity reactions can occur, including anaphylaxis. Stop this medication immediately if this occurs.

Caution in patients with narrow-angle glaucoma as it may increase intraocular pressure. 

Caution in patients with hyperthyroidism as it may stimulate thyroid activity.

Caution in patients with diabetes as it can cause an increase in serum glucose levels, although usually transient.

Caution in patients with a seizure disorder as use may result in CNS stimulation/excitation.

Caution in patients with hypokalemia as it may decrease serum potassium levels, although usually transient.

Caution in patients with cardiovascular disease as use of a beta-agonist has been reported to cause ECG changes. 

Caution in patients with prostatic hyperplasia/bladder neck retention due to potential urinary retention.

  • Cough, nasopharyngitis, bronchitis, upper respiratory tract infection, pharyngitis, respiratory insufficiency, sinusitis, dyspnea

Interactions

Does this medication interact with other drugs, foods, or alcohol?

Avoid use with aclidinium, anticholinergic agents, non-selective beta-blockers, cimetropium, eluxadoline, glycopyrrolate, glycopyrronium, levosulpiride, oxatomide, potassium chloride, potassium citrate, revefenacin, tiotropium, and umeclidinium (Risk Category X).

How should this medication be stored?
