What is Basaglar? 

Basaglar is a prescription medication that is classified as a long-acting insulin analog.

Uses

What is Basaglar used for?

It is used to improve glycemic control in adult and pediatric patients with Type 1 or 2 diabetes.

How does it work?

It exerts its blood glucose lowering effects by increasing glucose metabolism. This is accomplished by increasing glucose uptake and also by inhibiting hepatic glucose production.

Directions

How do I take Basaglar ?

Basaglar should be taken as directed by your physician. Never change the dose without provider approval. Every time you give yourself a dose, check the pen to make sure it is dialed to the correct amount of units so you don’t receive too much or not enough of your insulin dose. Injections can be given at any time of the day but make sure to take at the same time every day. Injection sites include: upper arm, abdomen, or thighs; rotate injection sites in the area you choose, avoid injecting insulin into the same spot.

What dosages are available?

It is available as brand-only 100 units/mL pens.

What is the recommended dosage?

The recommended dose varies based on individual patient factors. Refer to your doctor for specific dosing recommendations.

What is the maximum dosage?

The pen can deliver a max of 80 units per injection but there is no recommended daily maximum dose.

What if I miss a dose?

If you forget to take Basaglar, take it as soon as you remember. If it is relatively close to when your next dose is due, wait until then and take a normal dose. Do not take an extra dose to make up for the missed dose.

Warnings & Precautions

What should you know before you take Basaglar?

Never share Basaglar pens with other patients even if you use different needles

There is a risk of hyperglycemia and hypoglycemia with this medication. Check blood sugars regularly or as advised by your doctor as hypoglycemia may be life-threatening especially when your dose is changed.

Always check the label on the insulin before giving yourself a dose to ensure it is the correct medication.

Severe-life threatening allergies such as anaphylaxis have been reported. If this occurs, immediately discontinue.

Patients with hypokalemia or have risk factors for hypokalemia may need to be monitored.

Fluid retention and heart failure cases have been reported when this insulin is used concomitantly with thiazolidines. Discontinue or reduce dose if signs of heart failure develop.

Interactions

Does this medication interact with other drugs, foods, or alcohol?

May interact with other drugs that affect glucose metabolism. The dose of insulin may need to be lowered and additional glucose monitoring is recommended.

When used with anti-adrenergic drugs, the usual symptoms of hypoglycemia may be blunted. Sweating is a symptom of hypoglycemia that is not blunted when a patient is taking anti-adrenergic drugs.

Storage

How should this medication be stored?

Unopened and opened Basaglar pens can be stored at room temperature (68°F to 77°F) for up to 28 days. Once opened, leave the pens at room temperature, do not refrigerate. Unopened pens can also be stored in the refrigerator (36°F to 46°F) until the expiration date listed on the box.

Pharmacist Tips

  • Roll the pen in between the palms of your hands to warm up the mixture if taken out of the refrigerator. This will help make the injection less painful.
  • Signs of hypoglycemia include: dizziness, sweating, confusion, fast heartbeat, and headache. If you experience any of these symptoms, promptly measure your blood sugar. Hypoglycemia is defined as blood sugars measuring < 70 mg/dL. 
  • The goal is to maintain your blood sugar between 80 and 130 mg/dL. 
  • Dispose of used needles properly in a sharps-container that can be sealed appropriately once full.
