What should you know before you take Seroquel XR (Quetiapine XR)?

There is an increased risk of death for elderly patients with dementia related psychosis treated with antipsychotic drugs, Seroquel XR is not indicated for these patients. Additionally there is an increased risk of cerebrovascular accidents in this population while taking 2nd generation antipsychotics.

There is an increased risk of suicidal thoughts and behaviors in short-term studies in patients 24 and younger, patients starting this medication should notify their provider if they notice any negative changes in their mood.

Patients taking antipsychotic medications have an increased risk of Neuroleptic Malignant Syndrome (NMS). This condition is very rare but can be fatal so it is important to recognize the signs and symptoms of presentation. If you experience a high fever and muscle stiffness concurrently you should contact your physician as soon as possible.

Atypical antipsychotic medications are prime to inducing metabolic changes that include hyperglycemia, dyslipidemia, and body weight gain. Patients who are also diabetic should exercise caution when starting this medication as their insulin requirements may change.

In clinical trials there was found to be a risk of neutropenia/leukopenia in patients taking atypical antipsychotics. Patients with existing neutropenia should be monitored frequently and should discontinue Seroquel XR if their ANC <1000.
Patients on other medications that prolong their QT interval should have their QT interval evaluated prior to initiating Seroquel XR as it can also increase the QT interval.

It is metabolized by CYP3A4, as a result, medications that are either inhibitors/inducers of CYP3A4 should be determined and the dose should be adjusted accordingly if concurrent use is necessary.

• If you experience a high fever, excessive sweating, rigid muscles, confusion and/or changes in your breathing, heart rate, and blood pressure while taking this medication you should call your healthcare provider right away.
• Do not discontinue this medication without first consulting your doctor, this medication can cause withdrawal effects (insomnia, headache, dizziness, irritability, nausea, vomiting).
