What is Neupro (Rotigotine)? 

Neupro (Rotigotine) is prescription medication that is classified as an anti-parkinson agent and dopamine agonist. 

Uses

What is Neupro used for? 

It is indicated for the treatment of Parkinson disease and moderate to severe primary restless legs syndrome.

How does it work?

It exerts its effects by acting as an agonist at the dopamine receptors in the brain.

Directions

How do I take Neupro (Rotigotine)? 

Apply the patch to a dry, clean, hairless area on healthy, intact skin (unbroken skin) on either the thigh, hip, flank, shoulder, abdomen, or upper arm at about the same time each day.

You will remove the patch right before use and press the patch firmly onto the skin for 30 seconds. The application sites need to be rotated on a daily basis. Do not apply the patch to the same area more than once every 14 days or apply to oily, irritated, or damaged skin, or areas where the skin will be rubbed by tight clothing. Do not expose the patch to external heat sources.

If the area of the skin that you will apply the patch to is hairy, shave the area at least 3 days before applying the patch. If the patch falls off, apply a new patch to a new site immediately.

What dosages are available?

Neupro (Rotigotine) is only available as a brand as a 24 hour transdermal patch (1 mg/24hr, 2 mg/24hr, 3 mg/24hr, 4 mg/24hr, 6 mg/24hr, 8 mg/24hr).

What is the recommended dosage?

The typical dose is 1 mg transdermally daily but take as directed by your doctor.

What is the maximum dosage?

The maximum recommended dosage is 6 mg/day.

What if I miss a dose?

Apply the patch as soon as possible if you have missed a dose. If it is almost time for your next dose, skip the missed patch and return back to your regular schedule. Never apply two patches at once.

Warnings & Precautions

What should you know before you take Neupro (Rotigotine)?

Dose-dependent application site reactions (can be severe) have been seen. Rotating the application site daily has been shown to decrease this risk.

Although rare, cases of pleural effusion, pulmonary infiltrates, pleural thickening, retroperitoneal fibrosis, pericarditis and/or cardiac valvulopathy have been reported in patients using ergot-derived dopamine agonists. 

Fluid retention and weight gain have been reported, primarily associated with peripheral edema in patients with Parkinson disease.

Psychotic-like behavior/hallucinations may occur. Avoid use in patients with preexisting major psychotic disorders.

Risk for melanoma is increased in patients with Parkinson disease. Drug causation or other contributing factor risks have not been established. 

Use of dopamine agonists in patients with Parkinson disease or restless legs syndrome have been associated with impulse control disorders.

Orthostatic hypotension may occur with use. 

Somnolence can occur with use. 

Caution in patients with preexisting dyskinesia. 

Caution in patients with pre-existing cardiovascular disease. 

Augmentation of restless legs syndrome can occur.

Remove patch before magnetic resonance imaging or cardioversion to avoid skin burns due to the aluminum in the patch.

The patch contains sodium metabisulfite, which can cause an allergic reaction in some. 

Taper treatment when discontinuing the medication. Do not abruptly stop.

Side Effects

What are the side effects of Neupro (Rotigotine)?

Some common side effects include:

  • Orthostatic hypotension, peripheral edema, systolic hypertension
  • Hyperhidrosis
  • Decreased serum glucose
  • Decreased hematocrit, decreased hemoglobin
  • Application site reaction 
  • Dizziness, drowsiness, fatigue, hallucination, headache malaise, sleep disorder
  • Arthralgia, asthenia, dyskinesia
  • Increased blood urea nitrogen
  • Erythema of skin, pruritus
  • Xerostomia, anorexia, constipation, diarrhea 
  • Nasopharyngitis

Interactions

Does this medication interact with other drugs, foods, or alcohol?

Avoid use with alizapride, amisulpride, bromperidol, methotrimeprazine, metoclopramide, and sulpride (Risk Category X).

How should this medication be stored?

Neupro (Rotigotine) should be stored between 20º – 25ºC (68º – 77ºF). Excursions are allowed to be stored between 15º – 30ºC (59º – 86ºF). Keep Neupro (Rotigotine) in the original packaging.
